TRANSITIONAL OUTPATIENT SERVICES (TOS)

A One-Of-A-Kind Treatment Experience

Transitional Outpatient Services (TOS) are one-on-one companion services that are unique to each individual served. Their transition may include adapting to a lifestyle that incorporates self-care, personal well-being and recovery.  Transitional services provide a treatment plan and action-oriented strategies that are specifically designed for the individual in their environment. Having a definite plan in place helps the client develop a sense of accountability. In conjunction with their coach the opportunity to begin a sober lifestyle supports the destigmatizing of the 12-step recovery model; promotes and builds self-esteem, increases the client’s self-confidence and personal sense of well-being. 


PURPOSE:

Transitional support services are to help clients reduce self-sabotage; educate them about their own triggers and establish a relapse prevention plan that promotes healthy living. With this one-on-one support, we have found that clients have a decrease in their symptoms of depression and anxiety. With ongoing support and improving self-concept, families gain a renewed sense of trust for their loved one. By creating a new daily routine that stresses self-care and personal responsibility long term contentment and serenity can be achieved.  

Transitional Outpatient Services (TOS) focuses on the following areas: 

  • PAWS

  • Lifestyle changes

  • Living and Coping skills

  • Recovery Capital development

  • Balance

  • Emotional regulation

  • Family involvement

  • Continuum of care

  • Coordinated medical and mental health referrals

  • Cooperative involvement with legal and vocational referents

  • De-stigmatization of the disease and recovery

HOW: 

TOS provides 1-on-1 comprehensive services for clients who are new or returning to recovery and are ready to return to their home environments and looking for extra support. The individual client and counselor work together by creating, implementing, and coordinating a comprehensive, active, relevant treatment plan addressing needs within their readiness for change, relapse prevention, and issues related to their recovery environments.

  

THE GOALS:

The primary concept behind TOS is for the client to gain insight and awareness of their honest and real motivations, commitment, and an understanding of the severity of their disease and the problems it has caused in their life. 

The short-term goals of TOS are to: 

  • Develop a sober support system.

  • Learn new and healthy coping skills.

  • Engage in life enhancing behaviors


The long-term goals of TOS are to:

  • Assist clients in maintaining an improved daily lifestyle.

  • Build support structures by connecting with others.

  • Repair interpersonal relationships and re-connect with family members.

  • Return to manageable employment

  • Develop new and healthy social connections. 

  • Maintain a healthy diet, while exercising regularly. 

  • Identify and move beyond a stressful lifestyle by maintaining emotional balance.

  • Identify and explore ways to maintain a sense of spiritual well-being.
